Please complete this form to begin the withdrawal process. The Admissions Department will be notified of your intent.

 
Before submission of this form, be advised:

LAKE CENTER CHRISTIAN SCHOOL CONTINUOUS ENROLLMENT AGREEMENT, ITEM 6 (Signed upon enrollment): I understand and agree that between March 1st and June 1st, an additional fee will be assessed for withdrawing, and that after June 1st, the family is obligated to pay 25% of the tuition per student for the coming school year. 

LAKE CENTER CHRISTIAN SCHOOL REFUND POLICY: Students withdrawing from LCCS after a quarter has started will owe tuition for the full quarter.  This amount will be calculated by dividing the total tuition by four.  Any unpaid tuition from previous quarters will also be due upon withdrawal.  Parents who withdraw their children and still owe a tuition balance may be turned over to collections.

    After the official withdrawal date, LCCS will send a more detailed form for you to complete. This will include a questionnaire about your experiences at Lake Center. We ask you to complete this because it is an important tool in evaluating and helping Lake Center continue to meet high standards of excellence.
